The Long Term Social Care Teams of Stockton Borough Council are looking at new ways of working in a joint partnership approach to provide support planning.
The idea is to invite those who have been assessed as needing long term support to come to a hub. The hub will include representatives from Stockton Borough Council to discuss services which are commissioned. However they are also keen to refer clients to community/ VCSE activities and services which will fulfil all needs which have been identified in the assessment.
It is hoped that this model will be piloted in Billingham starting in March for six weeks.
Stockton Welfare Advice Network (SWAN) and Catalyst have been involved in the discussions and are currently mapping services available in Billingham. Designated contacts at SWAN and Catalyst (listed below) would therefore be grateful if you could forward details of any service which is provided in Billingham. These are currently categorised according to the Five Ways to WellBeing, which are:
LEARNING – ranging from how to knit to IT classes
ACTIVE – exercise classes, circus skills, walking
CONNECT – anything that alleviates social isolation from Mens’ Sheds to Knit and Natter groups
GIVE – volunteering
NOTICE – walking, nature.
In addition, as part of the promotion, if you have any visuals about the service you provide (e.g. photos, YouTube videos) please let us know as these will be collated to promote what is available in Billingham.
