Daisy Chain Project is looking for talented individuals who are passionate about advocating for and supporting children and young people with learning disabilities and/or autism and their families.
Hours: 37.5 hours per week (ability to work evenings and weekends as required)
Salary: £21,892-£24,157 (depending on experience)
This post is for a fixed term of two years.
With the support of the Senior Specialist Keyworker the post holder will work within a small team to provide training, advocacy and support to children and young people with a learning disability and/or autism and their families.
The post holder will provide early intervention to families identified as in need of additional support and will also address any gaps in support experienced by families whose child is at risk of exclusion or hospital admission (the Dynamic Support Register).
This role is one of three available and is part of a two year pilot project commissioned by the CCG and will work across the Tees Valley & Durham.
