Eastern Ravens Trust are seeking a highly motivated and experienced individual to deliver tailored support services to young carers and their families in the Stockton-on-Tees Borough. This post involves holistic whole-family support, working in partnership and delivering positive outcomes for young carers.
Key tasks for this role include:
- Planning and supporting respite breaks for young carers during term time, evenings, holiday activities, one-to-one support and occasional weekend work and residential’s as required.
- Identify, address and review young carers and family’s needs including signposting and referring to external service providers.
- Working with and providing support to young carers and their families which can often involve having difficult conversations.
