Reaching Out Across Durham (ROAD)
ROAD is an employment programme in County Durham funded by the European Social Fund and the National Lottery through the Big Lottery Fund. Working in partnership with 11 voluntary and community sector delivery Partners who are specialists in employment, health, financial and digital inclusion and volunteering, ROAD will support 625 long term unemployed people into or towards employment.

The Pioneering Care Partnership (PCP) is currently seeking motivated and focused self-starters to join their successful team as a Health Trainer.

Salary: £17,957 (Band 3)
Hours: 37.5 hours per week
Location: The Workplace, Aycliffe Business Park, Heighington Lane, Newton Aycliffe, DL5 6AH
Post: Fixed Term Contract Until July 2019

Job Purpose:
To work in a range of settings in County Durham to support a caseload of Reaching Out Across Durham (ROAD) Participants and provide them with 1:1 and group support, helping them overcome health barriers to employment. Working closely with Navigators to enable participants to address their barriers and realize their aspirations.
To provide a range of support and advice you will help each participant develop a tailored health action plan and monitor progress.
Key tasks:

  • Managing a caseload and maintaining regular supportive contact with each person.
  • Creating personal health action/activity plans with clients.
  • Developing and delivering group activities aimed at improving health or wellbeing.
  • Working with partners and pro-actively signposting clients onto other services.

Key skills required:

  • The ability to independently travel across County Durham
