If you are aged 50 or over and live in Stockton-on-Tees, Five Lamps offer a FREE service to help support you after your return home from hospital.
Home from hospital project can support elderly on their discharge home with:
– Shopping, and assisted shopping
– Medication collection
– Transport and assisted support to appointments such as: doctors, dentist, opticians/hearing tests and other appointments required
– Social isolation support- emotional support or just a social drop in
– Getting people back out into the community
– Supporting with bills, benefits and form filling with referrals
And more.
This service is available Monday-Friday
