Pioneering Care Partnership is seeking a motivated and compassionate ‘Join the Dots Facilitator’ to join our successful team.
Salary: £19,917 per annum, pro rata
Hours: 15 hours per week
Fixed term until 31st March 2020
Macmillan Joining the Dots is an exciting partnership between the Durham County Council and Macmillan Cancer Support which will help develop new ways of working to make sure people affected by cancer, including patients, carers and relatives receive the support they need. It focuses on non-clinical support, covering money, housing, practical and emotional support, lifestyle and spiritual issues and aims to address concerns over gaps in support available across the county.

It would be desirable if you have either working or lived experience of cancer, and strong personal resilience. However there is a good support network in place to help you.
