You will work as part of the Community Mental Health Transformation agenda. This framework sets out that individuals will receive mental health care where and when they need it, addressing inequalities in health care and social factors of mental ill health.
You will work in with TEWV NHS Trust Community Mental Health Teams and other VCS organisations to maximise the individual’s opportunities to reach their own potential, identify what the person’s difficulties and barriers are, and work with a person to develop skills in self-care, self-management and improvements in overall health and quality of life.
The postholder must hold a level 3 or above in Health or Social Care qualification.
SALARY: £27,161.67 pro rata
