Pioneering Care Partnership is currently seeking a motivated and focused self-starter to join their successful team as a Wellbeing for Life Senior Practitioner.
Full-Time Post
Salary: £21,391 per annum
Hours: 37.5 hours per week
Job Purpose:
The aim of Wellbeing for Life Senior Practitioners will be to support the management of Wellbeing Practitioners, whilst encouraging participation and managing a caseload of clients aged 16 years and above to support them through behaviour and lifestyle changes and to assist them to achieve their goals.
Key tasks:
- To work with partners and existing groups to develop the referral pathways for support.
- To develop new services/activities/groups for the community based on identified health need.
- To promote activities, including managing events and publicity campaigns.
- To line manage staff and develop the staff model for the service based on identified competencies required to deliver the client journey.
- To manage a caseload of individuals/groups and maintain regular supportive contact with each person.
Key skills required:
- You should have experience of managing staff, using referral pathways to signposting, working in the community or voluntary sectors, delivering 1-2-1 and group workshop activities.
- You need to have a relevant vocational qualification to Level 3, excellent communication, organisational, time management and interpersonal skills and be a team player.

Part-Time Post
Salary: £17,957 per annum
Hours: 20 hours per week
Job Purpose:
Wellbeing Practitioners will work with individuals and vulnerable groups aged 16 years and above to encourage participation and manage a caseload of clients to support them through behaviour and lifestyle changes and to assist them to achieve their goals.
The Wellbeing Practitioner will work in a range of settings in geographical patch/location to support the development and delivery of focused outreach work and signpost/referral pathways.
Key tasks:
- To encourage and facilitate clients in developing a personal health plan.
- To source a holistic package of support to assist the development of each individual.
- To asset map the local areas and signpost clients to relevant services.
Key skills required:
You should have an understanding of the factors affecting health and wellbeing and working with people to make positive changes.
Ideally, you need to have a relevant vocational health qualification to Level 3, excellent communication and interpersonal skills and be a team player.
