Pioneering Care Partnership (PCP) is seeking to employ a motivated and focused self-starter to join their successful team.
Salary: £19, 219 per annum
Hours: 37.5 hours per week
Job Purpose:
To co-ordinate the delivery of the SSNP project across Stockton-on-Tees
Key tasks:
To provide personalized assessment, signposting, information and support to clients in Stockton-on-Tees
To liaise with GP’s, health professionals and other partners to support the delivery of effective referral and recruitment pathways.
Key skills required:
Minimum of 12 months experience of working in the community or voluntary sectors
Experience of preparing individual action/support plans
Experience of using referral pathways to signposting.
