Catalyst's health projects

Catalyst deliver and manage a number of health projects throughout Hartlepool and Stockton-on-Tees.

We facilitate Health Initiatives which are highly successful VCSE-led social prescribing programmes that are funded by the Hartlepool and Stockton-on-Tees Clinical Commissioning Group.

Our projects

Integrated Personal Commissioning (IPC) programme

Stockton-on-Tees was chosen from 1 of 9 demonstrator sites in England to implement the Integrated Personal Commissioning (IPC) programme. Catalyst was chosen as the Voluntary Development Agency to help deliver this national NHS England programme.

Community Health Ambassadors

Community Health Ambassadors is a project developed by the Hartlepool & Stockton-on-Tees Clinical Commissioning Group (HAST CCG) to help it effectively engage and involve local people in the planning, development and commissioning of NHS health services. Catalyst co-ordinate a team of volunteers who represent a wide range of communities across the area to spread key NHS messages and improve patient experience.

A Fairer Start

A Fairer Start was a pilot programme managed by Catalyst from 2014 – 2018 with partners from the NHS North Tees & Hartlepool Foundation Trust and Stockton Borough Council. This programme brought together over 150 multi-agency professionals and volunteers to develop skills, knowledge and behaviours relating to Early Years practice whilst also providing additional community services for Stockton Town Centre ward families.
